Intestinal Obstruction

1. Cardinal Symptoms (Ab pain, distension, vomiting, absolute constipation)

2. Level of Obstruction

  • Based on pain (around umbilicus – midgut)
  • Onset of vomiting and level of distention

3. Causes : Adhesions, Hernia

Ix

X-Ray Supine Abdomen

? Differentiating small from large bowel

Mx

Intially conservative.

  1. NG tube decompression
  2. IV fluid replacement

Clinical signs suggestive of impending strangulation.

  • Colicky pain –> Persistent pain
  • Poor localisation –> Localized pain
  • Pt deteriorating

Ex Lap if risk of strangulation.

Escharatomy

  • For circumferential, full-thickness burns.
  • Tourniquet effect.
  • Dressing for full thickness burns
  • 1% Silver Sulphadiazine cream
