Finding the Tissue Plane

  • Lump moves with skin – Attached to skin (Sebaceous cyst)
  • If skin above the lump is movable (by 2 fingers or pinching over the lump) – Lump is not attached to the skin.
  • Then hold the lump and try to move it. If immobile and hard – Bony lump (Osteoma).
  • Ask to flex the muscle. If mobility reduces – Muscle plane
  • If mobility doesn’t reduce – Subcutaneous plane

 

Sebaceous Cyst

Salient features

  1. Skin discolouration
  2. Punctum
  3. Attached to the skin
  4. Indentation (when lump is pressed indentation retains)

Excision under LA.

Can get infected.
